The Therapeutic Practitioner provides intensive, trauma-informed support to young people aged 18–22 who are experiencing homelessness, mental health challenges, AOD issues, and trauma. You will work alongside social workers and the in-house clinician to deliver tailored support plans, provide secondary consultation, and engage young people in meaningful activities to build trust, stability, and long-term outcomes.
Responsibilities:
Deliver therapeutic support to young people in long-term housing and transitional programs
Provide secondary consultation to social workers and other team members
Support engagement through flexible, creative, and relational approaches (e.g., attending social activities)
Maintain accurate, high-quality documentation in line with organisational policy and legislation
Collaborate with multidisciplinary teams, external agencies, and in-house clinicians
Participate in case conferencing, care planning, and team reflective practice sessions
Conduct outreach and home visits as required
Requirements:
Qualifications in psychology, social work, or therapy preferred
Experience working with young people aged 18–25, particularly in leaving care, child protection, or corrections contexts
Demonstrated trauma-informed practice and ability to engage complex presentations
Strong interpersonal and communication skills
Valid Victorian driver’s license and willingness to travel locally for outreach
